Multi-State Nurse Practitioner

Job Summary

To support a growing remote care initiative, the contracted Multi-State Nurse Practitioner will provide diabetes education and management through phone consultations, evaluate member health data, and prescribe continuous glucose monitors while maintaining a flexible schedule of 15-20+ hours per week.

Key Responsibilities

• Provide call-based care to members in the Remote Patient Monitoring (RPM) program
• Evaluate medical history and data to determine member eligibility for the clinical program
• Collaborate with the care team to reinforce clinical goals and ensure quality compliance

Required Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution; active Nurse Practitioner certification
• Valid, unencumbered state nurse practitioner license with multi-state licensure required
• Minimum 3+ years of experience managing patients with diabetes
• Familiarity with continuous glucose monitors (CGMs) is a plus
• Spanish-speaking highly preferred
