Multi-State Nurse Practitioner Reviewer

Job Summary

To enhance patient care through effective reporting, the full-time Nurse Practitioner Clinical Reviewer will review Remote Patient Monitoring, Behavioral Health, and Chronic Care Management reports, manage escalated alerts, and collaborate with clinical teams, all while working remotely.

Key responsibilities

• Review and ensure the integrity of patient reports related to Remote Patient Monitoring and Chronic Care Management
• Contact facility clinical leadership to obtain feedback on reports and educate them on their utilization
• Analyze escalated alerts from the Passive Device Alert Center and notify appropriate facility contacts of high-risk situations

Required qualifications

• Advanced Practice Nurse practitioner education and certification
• Active NP licenses in at least 5 states
• Knowledge and skills in chronic disease and remote patient monitoring management
• Proficiency with Adobe, Microsoft Word, Excel, and video conferencing platforms
• Ability to work independently and maintain a positive attitude
