Job Summary

A company is looking for a Registered Nurse - Health Coach Consultant.

Key Responsibilities

• Conduct clinical assessments and engage members in chronic condition support and personalized coaching
• Provide individualized telephonic health coaching and promote sustainable behavior change
• Develop and maintain individualized care plans, document interactions, and monitor member progress

Required Qualifications

• Must hold an unrestricted nursing license and reside in a compact state with multi-state privileges
• 3+ years of acute adult Medical-Surgical RN experience
• High-speed, hard-wired internet connection
• Proficiency with Microsoft Office and EMR documentation experience
• Associate Degree in Nursing (ADN) required; BSN preferred
