Multistate Licensed Nurse Practitioner

Job Summary

To support a transformative healthcare initiative, the full-time remote Multistate Licensed Nurse Practitioner will conduct telehealth visits, optimize patient medical therapy for chronic conditions, and collaborate with healthcare providers to enhance patient outcomes.

Key responsibilities

• Conduct synchronous telehealth visits to assess and manage patients with chronic diseases
• Order necessary lab tests and provide treatment recommendations to referring providers
• Review remote patient monitoring data and achieve clinical outcomes for a panel of patients

Required qualifications

• Minimum of 3+ years of experience treating patients with cardiovascular, cardiometabolic, or related conditions in an outpatient setting
• Active multistate RN license and licensure as an Advanced Practice Nurse in at least 5 states
• Completion of an accredited graduate-level APRN program
• National certification as an APRN with an Adult, Adult-Gerontological, or Family focus
• Current enrollment as a Medicare Part B provider
