Multistate Licensed RN Case Manager

Job Summary

Managing complex member caseloads, the full-time Multistate Licensed RN Case Manager will conduct telephonic assessments, develop individualized care plans, and coordinate services for Medicaid populations, ensuring quality care and continuity in a remote environment.

Key responsibilities

• Complete comprehensive telephonic assessments and develop individualized care plans addressing diverse member needs
• Coordinate clinical services with providers and community organizations to facilitate continuity of care and support safe transitions
• Document assessments and care interventions accurately in the electronic health record (EHR) while ensuring compliance with quality standards

Required qualifications

• Active multistate Registered Nurse (RN) license required
• CCM certification preferred
• Minimum 2+ years of experience in care management, case management, or population health
• Experience supporting Medicaid populations or complex care environments preferred
• Experience using electronic health record (EHR) systems
