Multistate Licensed Telehealth Physician

Job Summary

To support a growing telehealth initiative, the remote contract Multistate Licensed Telehealth Physician will conduct structured intake consultations to establish medical necessity for care navigation programs, identify high-risk conditions, and validate care plans.

Key responsibilities

• Conduct telehealth-based E/M or Annual Wellness Visits (AWVs) to establish medical necessity for care navigation services
• Document findings accurately in the platform, ensuring proper coding and use of SDOH Z-codes
• Develop and approve individualized care plans aligned with patients' medical treatment needs

Required qualifications

• M.D. or D.O. with an active, unrestricted U.S. medical license
• Broad multistate licensure strongly required (IMLC participation or 50-state coverage preferred)
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• 2+ years of experience in primary care, telehealth, or care management
• Familiarity with Medicare programs and proficiency with EMRs and telehealth workflows
