Job Summary

A company is looking for a Manager, Care Management.

Key Responsibilities

• Manage and develop a team of Team Leads and Care Coordinators to ensure high-quality patient-centric care
• Drive operational effectiveness by monitoring quality and key performance indicators
• Lead change management initiatives and process improvements across workflows and protocols

Required Qualifications

• Active multistate/compact RN license preferred
• Bachelor's or Master's in Nursing or Healthcare preferred
• Certification in Case Management (CCM) preferred
• 3-5 years of direct leadership experience in care management
• Experience in integrated Care Management or telehealth, particularly in Chronic Care Management or Remote Patient Monitoring
