Multistate RN Case Manager

Job Summary

A company is looking for a Case Manager (RN).

Key Responsibilities

• Lead coordination of a multidisciplinary team to provide holistic care to members via telephonic and digital communication
• Utilize the case management process to assess, develop, implement, monitor, and evaluate individualized care plans
• Advocate for members and arrange necessary resources to meet identified health and social needs

Required Qualifications, Training, and Education

• Nursing Diploma or Associate's degree in nursing required; Bachelor's degree strongly preferred
• 3 years of clinical nursing experience in various healthcare settings required
• 1 year of case management experience in a managed care setting strongly preferred
• Current, active, and unrestricted Multistate Compact Registered Nurse license required
• Certification in Chronic Care Professional (CCP) and Certification in Case Management preferred
