Multistate RN Case Manager

Job Summary

Providing proactive patient and provider outreach, the full-time remote Multistate RN Case Manager will focus on ED discharge follow-up care, coordinating transitions of care in collaboration with healthcare teams and ensuring continuity of care for members.

Key responsibilities

• Independently collaborates with ED and outpatient care teams to create individualized transition plans for members
• Serves as the clinical liaison, performing transition of care and care coordination for ED discharges using evidence-based criteria
• Conducts outreach post-ED discharge to confirm access and completion of discharge plans

Required qualifications

• Current, unrestricted Multistate RN license
• 4+ years of diverse clinical experience in caring for acutely ill patients
• 2+ years of experience working in the Emergency Department
• Knowledge of utilization management, quality improvement, and discharge planning
• Ability to provide support 24/7
