Job Summary

A company is looking for a Population Outcomes Manager for the East Coast.

Key Responsibilities

• Identify and analyze utilization trends and collaborate with interdisciplinary teams to plan interventions
• Analyze inpatient admissions to identify preventable utilization and develop strategies to avoid future occurrences
• Design, implement, and manage projects to ensure effective interventions and track outcomes

Required Qualifications

• Current multistate RN License in good standing
• Bachelor's degree or equivalent work experience; Master's degree preferred
• Four or more years of clinical experience, including at least 2 years in a value-based care environment
• Geriatric care and population health experience highly desired
• Experience in report generation and data analysis, particularly in Excel
