Job Summary

A company is looking for a Health Coach responsible for implementing personalized health plans and supporting clients in behavior change.

Key Responsibilities

• Partner with physicians to implement personalized health plans based on various data sources
• Conduct telemedicine coaching sessions focusing on nutrition, exercise, sleep, and stress management
• Track client progress and communicate regularly with care teams to ensure continuity of care

Required Qualifications

• National Board Certified Health & Wellness Coach (NBC-HWC) certification
• Completion of an NBHWC-approved training program
• Minimum 3 years of professional health coaching experience in clinical settings
• Strong understanding of behavior change science and motivational interviewing
• Ability to work independently and collaboratively in a remote environment
