Nebraska Licensed Maternity Case Manager

Job Summary

A company is looking for a Bi-lingual Maternity Case Manager, LCSW (Remote) NE license.

Key Responsibilities

• Empower caregivers and families to support optimal wellness and advocate for necessary services
• Assess, plan, and implement individualized care strategies directed toward appropriate levels of care
• Build relationships with caregivers and healthcare providers to develop member-focused care plans

Required Qualifications

• Licensed in OH, MI, NY, and/or IN
• Minimum of 2 years of experience in Pediatrics/NICU managed care or clinical environment
• Valid unrestricted Social Worker license in state of residence, with willingness to obtain additional licenses as needed
• Five or more years of managed care experience in a case management role for telecommuting positions
• Experience with Medicaid populations and knowledge of social determinants of health preferred
