Job Summary

A company is looking for a Care Coordinator specializing in needs assessment.

Key Responsibilities
  • Inform families about program options and services available
  • Maintain accurate and up-to-date candidate information in the database
  • Assist families in making informed decisions regarding clinical service next steps
Required Qualifications, Training, and Education
  • Bachelor's degree with a minimum of 3 years of experience in human services or a related clinical field
  • Experience supporting families with children on the autism spectrum or with developmental disabilities
  • Ability to conduct needs assessments and interviews effectively
  • Proficient in various software programs, including database management and Microsoft Office
  • Fluency in French is preferred; knowledge of other languages and experience with Indigenous families is an asset
