Needs Assessment Coordinator

Job Summary

To support client-focused care, the remote Needs Assessment Coordinator will conduct screenings on potential client calls, complete necessary documentation and reports, and manage on-call admissions and referrals.

Key responsibilities

• Conduct screenings on potential client calls and complete associated documentation
• Complete all required reports related to Needs Assessment and Referral Development
• Manage on-call admissions, referrals, and transportation needs during off-hours

Required qualifications

• High School Diploma or equivalent required; Bachelor's Degree in a Human Service field preferred
• Experience in a health or human services agency, preferably in a drug & alcohol setting
• Working knowledge of emotional, mental health, and drug & alcohol issues
• Demonstrated ability to maintain confidentiality and safety
• Effective interpersonal skills and ability to work as a team player
