Job Summary

A company is looking for a Remote Bilingual LPN Care Coordinator.

Key Responsibilities

• Serve as the primary point of contact for patients in chronic care programs
• Deliver telephonic or virtual outreach and educate patients on medication and lifestyle changes
• Coordinate care with pharmacists and nurses while ensuring documentation meets quality standards

Required Qualifications

• Active Nevada LPN or RN license in good standing
• Fluency in Spanish and English
• Minimum 2 years of clinical or telehealth experience, preferably in chronic disease management
• Familiarity with CCM and/or RPM workflows is an advantage
• Ability to work independently in a secure, HIPAA-compliant remote environment
