Nevada Licensed Care Coordinator

Job Summary

A company is looking for an LPN-Care Coordinator to work remotely, coordinating transitions of care for TRICARE beneficiaries.

Key Responsibilities

• Coordinate transitions of care for TRICARE beneficiaries through inbound and outbound calls
• Communicate with community providers and facilities to ensure care coordination and authorizations are in place
• Document coordination activities accurately in the Medical Management System (MMS) and participate in quality improvement projects

Required Qualifications

• Current unrestricted Licensed Practical or Vocational Nurse (LPN/LVN) license in the appropriate state or territory
• Minimum of 2 years' clinical nursing experience; preferred 3 years' clinical experience and 2+ years' Managed Care experience
• Proficiency in medical data entry and Microsoft Office
• Utilization of clinical and analytical skills for care coordination needs
• Ability to work remotely with regular and reliable attendance
