Job Summary

To support a growing team dedicated to high-acuity Medicare Advantage members, the full-time Nevada Licensed Complex Care RN will coordinate care across disciplines, manage transitions of care, and ensure timely clinical monitoring in a fully remote environment.

Key responsibilities:

• Own the member journey and care coordination for high-acuity members, maintaining consistent engagement and proactive monitoring of clinical status
• Manage transitions of care and hospital discharge coordination, ensuring timely outreach and follow-up to reduce avoidable readmissions
• Complete medication reconciliations and clinical monitoring, identifying potential interactions and escalating concerns to the appropriate care providers

Required qualifications:

• Minimum 3 years of clinical RN experience in complex care, care management, or related settings
• Active, unrestricted Registered Nurse (RN) license in Nevada
• Experience in a Medicare Advantage or managed care environment with knowledge of HEDIS and care gap management
• Proficiency in telehealth delivery and virtual member engagement
• Associate Degree in Nursing (ADN); Bachelor of Science in Nursing (BSN) strongly preferred
