Job Summary

A company is looking for a Complex Discharge Planner (RN).

Key Responsibilities

• Develops and coordinates complex discharge plans for high-risk members based on needs assessments
• Interacts with providers and community agencies to ensure safe and effective transition plans
• Educates members and their families on discharge plans and maintains documentation in health management systems

Required Qualifications, Training, and Education

• Graduate from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience, with 2+ years of acute care experience required
• Clinical knowledge of discharge planning competencies preferred
• Knowledge of Medicare and Medicaid regulations preferred
• RN - Registered Nurse - State Licensure and/or Compact State Licensure for the state of Nevada required
