Job Summary

A company is looking for a Clinical Navigation, Family Nurse Practitioner (Per Diem).

Key Responsibilities:

• Assess patients' needs, gather clinical history, and provide clinical guidance while navigating them to appropriate resources
• Deliver high-quality clinical services across various Included Health service lines and monitor care quality
• Evaluate and manage patients with acute, chronic, and mental health conditions, collaborating on projects to improve efficiency and member experience

Required Qualifications:

• Licensed NP/PA provider in good standing with state board
• Maintains board certification
• At least 3 years of NP/PA experience in a clinical setting
• Comfortable providing care to all age groups
• Must be in PST or MST time zone
