Nevada Licensed Mom and Baby Care Manager

Job Summary

To enhance care delivery, the full-time salaried Nevada Licensed Mom and Baby Care Manager will collaborate with interdisciplinary teams and community organizations to develop and implement person-centered care plans, conduct health assessments, and empower members in managing their health and wellness, all while working remotely.

Key responsibilities

• Engage members through strength-based assessments and develop individualized care plans in collaboration with the interdisciplinary care team
• Facilitate communication and coordination of care among members, their support systems, and healthcare providers to achieve health goals
• Monitor member satisfaction and adjust care interventions based on progress and changing needs

Required qualifications

• Bachelor's degree in a healthcare field or equivalent relevant work experience
• A minimum of three years of experience in social work, counseling, or related healthcare roles
• Current unrestricted clinical license in the state of practice as a Registered Nurse, Social Worker, or Professional Clinical Counselor
• Experience with Medicaid and/or Medicare managed care is preferred
• Advanced degree associated with clinical licensure is preferred
