Job Summary

A company is looking for a Pediatric/OB Social Worker.

Key Responsibilities

• Identify and initiate referrals for social service programs, including financial and community services
• Manage the social service plan of care throughout the continuum of care as a single point of contact
• Document member assessments and services provided, ensuring follow-up on referrals

Required Qualifications

• Bachelor's Degree in Social Work required
• State of Nevada Social Work license required
• Minimum of two years of related social work experience in an acute care or case management setting
• Working knowledge of social work theory, diagnostic tools, and community-based services
• Understanding of managed care preferred
