Job Summary

A company is looking for a Pharmacist to work from home on a contract basis.

Key Responsibilities

• Engage members and schedule pharmacist consultation appointments
• Ensure patient privacy and confidentiality in compliance with HIPAA regulations
• Utilize prescription claims data to provide clinical recommendations and assess medication regimens

Required Qualifications

• Bachelor's Degree in Pharmacy or PharmD
• Current and unrestricted Pharmacist license in state of residence
• Must reside within PST or MST time zones
• Strong knowledge of disease state management guidelines and MTM services
• Ability to navigate multiple databases and applications effectively
