Nevada Licensed Practical Nurse

Job Summary

Seeking a compassionate, bilingual Nevada Licensed Practical Nurse (LPN/LVN) for a fully remote contractor position, where the nurse will provide education, care coordination, and clinical support to patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.

Key Responsibilities

• Conduct comprehensive patient assessments to identify clinical, educational, and psychosocial needs
• Provide patient education in both Spanish and English regarding chronic disease management and self-management strategies
• Coordinate care across providers and monitor Remote Patient Monitoring (RPM) data to ensure continuity of care

Required Qualifications

• Active, unrestricted Nevada Licensed Practical Nurse (LPN/LVN) license in good standing or willingness to obtain one
• Minimum of 2 years of clinical nursing experience, preferably in primary care or chronic disease management
• Fluent in both Spanish and English, capable of effective communication with diverse patient populations
• Strong knowledge of chronic disease management and patient education principles
• Experience using Electronic Health Records (EHR) and healthcare technology platforms
