Job Summary
Providing telephonic care and coordination, the full-time New England Licensed Nurse Practitioner will manage patient interactions across various settings, ensuring quality care delivery while working remotely with a flexible schedule that includes evenings, nights, and rotating weekends.
Key responsibilities:
- Deliver telephonic and telehealth care to patients, coordinating with caregivers and facilities
- Conduct comprehensive assessments and document findings in compliance with regulations
- Coordinate care transitions and facilitate adjustments to care plans as needed
Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states (NY, NJ, CT, ME, NH, RI)
- Board certification through a national board in relevant Nurse Practitioner specialty
- Current DEA licensure or ability to obtain post-hire as per state regulations
- Proficient in using electronic medical records for documentation
