Job Summary
Providing telephonic care to patients, the part-time New England Licensed Nurse Practitioner will manage calls and coordinate care for members while working remotely, requiring evening, night, and weekend availability.
Key responsibilities:
- Deliver telephonic care and manage patient interactions across various healthcare settings
- Conduct comprehensive assessments and document findings in compliance with regulations
- Coordinate care transitions and facilitate adjustments to care plans as needed
Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New England states including New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island
- Board certified through a national board in relevant Nurse Practitioner specialties
- Current DEA licensure or ability to obtain post-hire, per state regulations
- Proficient computer skills for documenting medical information in electronic medical records
