New Hampshire Licensed Care Manager

Job Summary

To support the NH Health Families Medicaid population, the full-time New Hampshire Licensed Care Manager will develop and facilitate complex care management activities, focusing on physical health needs while working remotely.

Key responsibilities

• Evaluate member needs and barriers to care, recommending personalized care plans and interventions
• Develop ongoing care plans in collaboration with providers and community resources to address unmet needs
• Provide education to members and their families on healthcare options, disease processes, and care management

Required qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Current RN licensure in New Hampshire or Compact State
• Experience in both Physical Health and Behavioral Health is highly preferred
