Job Summary

A company is looking for a Case Manager - Behavioral Health - Full Time.

Key Responsibilities:

• Coordinate and provide safe, effective, and client-focused care
• Monitor and evaluate client progress through case management plans
• Educate and support patients transitioning to community-based services

Required Qualifications, Training, and Education:

• Master's degree in Social Work, Mental Health Counseling, or related field
• Experience in a social service setting with knowledge of case management systems
• Ability to assess crisis situations and respond appropriately
• Familiarity with documentation and compliance with guidelines
• Strong understanding of mental health and substance abuse disorders
