Job Summary
Providing clinical social work interventions, the full-time New Hampshire Licensed Case Manager will assess treatment needs, develop and monitor treatment plans, and empower clients to achieve their goals, working onsite three days a week and remotely two days a week.
Key responsibilities:
- Coordinate and provide safe, effective, and client-focused care across the continuum
- Monitor and evaluate client progression through case management plans, making necessary adjustments
- Educate and support patients in transitioning to community-based services
Required qualifications:
- Master's degree in social work, Mental Health Counseling, or a related field
- Experience in a social service setting with knowledge of case management systems
- Ability to assess crisis situations and respond appropriately
- Strong documentation skills in accordance with Federal and State guidelines
- Effective communication skills with diverse individuals and backgrounds
