Job Summary

A company is looking for a Clinical Assessor (PRN) Remote within the State of New Hampshire.

Key Responsibilities

• Conduct needs-based eligibility assessments for Medicaid-funded personal care services
• Ensure service authorizations align with beneficiaries' medical conditions and functional limitations
• Complete various assessments and maintain compliance with corporate policies and HIPAA regulations

Required Qualifications and Experience

• Active and unrestricted Registered Nurse (RN) license from the State of New Hampshire or multistate compact RN license
• Minimum of two years of experience in a home care or community-based clinical setting
• Proficiency in medical record abstracting and understanding of medical terminology
• Ability to communicate effectively through various media
