Job Summary
To enhance patient care through personalized support, the remote New Hampshire Licensed Clinical Pharmacist will manage medication therapy, develop clinical protocols, and monitor patient adherence while collaborating with healthcare professionals.
Key responsibilities
- Review and interpret patient care notes to support prior authorization approvals and care plan recommendations
- Monitor patient adherence and evaluate therapies, recommending drug therapy changes as necessary
- Oversee daily activities of support personnel and ensure compliance with HIPAA regulations
Required qualifications
- Doctor of Pharmacy (PharmD) or equivalent advanced degree
- Current New Hampshire pharmacist license
- 1+ years of experience in a clinical pharmacy setting preferred
- Strong knowledge of specialty medication pharmacology and pharmacy regulations
- Proficiency in Microsoft Office tools and database applications
