Job Summary

A company is looking for a Telephonic LPN - Care Coordination (Full Remote).

Key Responsibilities

• Conduct telephonic outreach to coordinate care, admissions, and services for members
• Support transitions of care for members entering or discharging from long-term care or rehabilitation settings
• Gather and review clinical documentation and assist with prior authorization requests

Qualifications

• Active, unrestricted LPN license in the State of New Hampshire (required)
• Minimum 2 years of nursing experience in long-term care, home health, or care coordination
• Familiarity with New Hampshire Medicaid processes preferred
• Proficiency in EMR systems and care coordination technology
• Ability to work independently in a remote setting
