Job Summary

A company is looking for a Telephonic LPN Care Coordination.

Key Responsibilities

• Conduct telephonic outreach to coordinate care, admissions, and services
• Support transitions of care for members entering or discharging from long-term care or rehabilitation settings
• Document all interactions and ensure compliance with HIPAA and organizational protocols

Qualifications

• Active, unrestricted LPN license in the State of New Hampshire
• Minimum 2 years of nursing experience in long-term care or care coordination
• Familiarity with New Hampshire Medicaid processes preferred
• Proficiency in EMR systems and care coordination technology
• Ability to work independently in a remote setting
