New Hampshire Licensed Neurology Nurse Practitioner

Job Summary

To enhance patient access and reduce time to treatment, the full-time New Hampshire Licensed Neurology Nurse Practitioner will provide remote clinical care to patients with neurological conditions and collaborate with physicians and the care team.

Key responsibilities

• Provide direct clinical care to patients via telehealth, focusing on neurological conditions and memory disorders
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
• Educate patients and families on diagnoses, treatment plans, and disease progression while maintaining accurate clinical documentation

Required qualifications

• Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgment and clinical documentation skills
• Ability to manage a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
