New Hampshire Licensed Nurse Practitioner

Job Summary

To support a growing virtual care team, the full-time New Hampshire Licensed Nurse Practitioner will provide direct clinical care to patients with neurological conditions and memory disorders via telehealth, collaborating with physicians and the care team while maintaining accurate clinical documentation.

Key responsibilities

• Provide direct clinical care to patients with neurological conditions and memory disorders via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
• Educate patients and families on diagnoses, treatment plans, and disease progression while ensuring continuity of care

Required qualifications

• Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgment and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
