Job Summary
To support eligible members in achieving optimal health, the part-time New Hampshire Licensed Transition Care Manager will assess needs, develop individualized care plans, and coordinate services remotely while ensuring compliance with health plan requirements and regulations.
Key responsibilities
- Assess members' care coordination needs through face-to-face encounters and telephone communications
- Create and implement comprehensive care plans to address barriers and achieve health goals
- Coordinate physical, behavioral health, and social services while documenting progress in the Electronic Medical Record (EMR) system
Required qualifications
- Current and unrestricted Registered Nurse (RN) license or Licensed Master Social Worker (LMSW)/Licensed Clinical Social Worker (LCSW) license
- 3 to 5 years of experience in case management and transition of care planning
- Experience working with individuals with complex needs, preferably in a managed care organization
- Proficiency with Microsoft Tools and EMR systems
