Job Summary

A company is looking for a MLTSS Clinical Care Associate.

Key Responsibilities:

• Assist with scheduling and coordination of services in the member's Plan of Care
• Provide telephonic assistance and follow-up to ensure timely member engagement
• Document interactions and coordinate the delivery of care based on members' needs

Required Qualifications:

• High School Diploma/GED required
• Preferred healthcare industry experience
• Minimum one (1) year experience in home care, discharge planning, or case management
• Minimum one (1) year experience in the managed care industry preferred
• Knowledge of managed care principles and standards of practice for care managers
