Job Summary
To support community-based housing initiatives, the full-time New Jersey Housing Specialist will develop and implement housing strategies for MLTSS/SNP/Medicaid members, manage housing resources, and provide education to staff while working remotely.
Key responsibilities
- Develop a housing strategy for MLTSS/SNP/Medicaid membership
- Identify and manage resources for housing options and assist with member placements
- Provide education to care management and advocacy staff on housing resources and options
Required qualifications
- High School Diploma/GED required
- Three years of experience assisting vulnerable populations in securing housing
- Two years of experience in a public agency or related social service field
- Familiarity with public and private housing resources and stakeholders
- Knowledge of Medicare/Medicaid and Managed Long-Term Services and Supports (MLTSS)
