New Jersey Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator Specialist to provide coordinated care services for injured workers under RN supervision.

Key Responsibilities

• Coordinate routine medical care needs and facilitate communication among all parties involved
• Monitor adherence to treatment plans and maintain accurate documentation in the workers' compensation system
• Support administrative functions, including scheduling appointments and attending team meetings

Required Qualifications

• High School Diploma required
• Graduate of an accredited practical/vocational nursing program
• Current compact LPN/LVN license in good standing
• 2+ years of clinical experience required, preferably in workers' compensation or disability management
• Experience in healthcare coordination and documentation preferred
