Job Summary

A company is looking for a Care Manager I, Specialty Programs.

Key Responsibilities:

• Assess member's clinical needs against established guidelines for specialty populations
• Implement high-quality, cost-effective care based on customized population models and clinical practice guidelines
• Coordinate with members, families, and healthcare teams to ensure comprehensive care and resource utilization

Required Qualifications:

• License as an RN or a Bachelor's degree in social work, health, or behavioral science
• Graduate level degree in Social Work or Health and Behavioral Science preferred
• Minimum of three (3) years' experience serving members with special needs
• Strong knowledge of case management standards and managed care principles
• Knowledge of specialty populations as applicable
