New Jersey Licensed Care Manager

Job Summary

A company is looking for a Care Manager I, Specialty Programs-Medicaid, Care, Case and Disease Management.

Key Responsibilities

• Assess clinical needs of members against established guidelines for specialty populations
• Implement high-quality, cost-effective care based on customized population models and clinical guidelines
• Coordinate resources and advocate for members among healthcare teams to ensure their needs are met

Required Qualifications

• License as an RN or Bachelor's degree in social work, health, or behavioral science
• Graduate level degree in Social Work or Health and Behavioral Science preferred
• Minimum of three (3) years' experience serving members with special needs
• Strong knowledge of case management standards and managed care principles
• Knowledge of specialty populations as applicable
