Job Summary

A company is looking for a Care Manager I, Specialty Programs (Childhood Lead Program).

Key Responsibilities

• Assess member's clinical needs against established guidelines for specialty populations
• Implement high-quality, cost-effective care based on customized population models
• Coordinate resources and advocate for members among healthcare teams and external services

Required Qualifications

• License as an RN or a Bachelor's degree in social work, health, or behavioral science
• Graduate level degree in Social Work or Health and Behavioral Science preferred
• Minimum of three years' experience serving members with special needs
• Strong knowledge of managed care principles and standards of practice for case managers
• Knowledge of specialty populations as applicable
