Job Summary

A company is looking for a Care Manager I, Specialty Programs.

Key Responsibilities

• Assess member's clinical needs and implement care based on established guidelines for specialty populations
• Coordinate care with members, families, and healthcare teams to ensure needs are met and advocate for resource utilization
• Document care activities accurately and participate in quality improvement initiatives to enhance member outcomes

Required Qualifications

• Requires a license as an RN or a Bachelor's degree in social work, health, or behavioral science
• Graduate level degree in Social Work or Health and Behavioral Science preferred
• Minimum of three (3) years' experience serving members with special needs
• Strong knowledge of case management standards and managed care principles
• Strong knowledge of specialty populations as applicable
