New Jersey Licensed Care Manager

Job Summary

To support specialty populations, the full-time New Jersey Licensed Care Manager will perform care coordination duties remotely, ensuring appropriate levels of care and planning for transitions while advocating for members' needs.

Key responsibilities

• Assess clinical needs of members against established guidelines for specialty populations
• Implement high-quality, cost-effective care based on customized population models and clinical practice guidelines
• Partner with members, families, and healthcare teams to coordinate resource utilization and monitor care activities

Required qualifications

• New Jersey RN license or Bachelor's degree in social work, health, or behavioral science
• Graduate-level degree in Social Work or Health and Behavioral Science preferred
• Minimum of three years' experience serving members with special needs
• Strong knowledge of managed care principles and standards of practice for case managers
• Valid New Jersey State driver's license and insurance, as required for certain positions
