New Jersey Licensed Care Manager

Job Summary

To support patients enrolled in virtual chronic care and population health programs, a compassionate and highly organized New Jersey Licensed Care Manager (LPN/LVN) will engage in telephonic and virtual care encounters while working remotely on a flexible schedule.

Key Responsibilities

• Engage patients through scheduled telephonic and virtual care encounters
• Support patients in Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) programs
• Assist with care coordination activities, including referrals and follow-up appointments

Required Qualifications

• Active New Jersey LPN license in good standing
• Minimum two years of nursing experience in relevant clinical settings
• Proficiency with electronic health records and web-based clinical platforms
• Ability to work independently while maintaining productivity and quality expectations
• Dedicated HIPAA-compliant home workspace with reliable internet access
