New Jersey Licensed Care Manager

Job Summary

To support the Childhood Lead Program, the full-time New Jersey Licensed Care Manager will perform care coordination duties, assess clinical needs, and implement interventions to ensure efficient utilization of benefits while working remotely.

Key responsibilities

• Assess members' clinical needs against established guidelines for specialty populations
• Implement high-quality, cost-effective care based on customized population models and clinical practice guidelines
• Collaborate with members, families, and healthcare teams to advocate for and coordinate resource utilization for members

Required qualifications

• New Jersey RN license or bachelor's degree in social work, health, or behavioral science
• Graduate level degree in Social Work or Health and Behavioral Science preferred
• Minimum of three years' experience serving members with special needs
• Strong knowledge of managed care principles and standards of practice for case managers
• Valid New Jersey State driver's license and insurance may be required for certain positions
