Job Summary

Collaborating with interdisciplinary teams, the full-time New Jersey Licensed Care Progression Manager will oversee patient care progression, ensuring timely discharge planning and alignment with value-based care initiatives in a hybrid work environment.

Key responsibilities
  • Oversee patient populations to ensure timely care progression and safe discharge planning using evidence-based criteria
  • Identify clinical conditions and appropriate interventions for patients in the ambulatory space
  • Monitor medical necessity and collaborate with clinical teams to resolve barriers to care and improve patient outcomes
Required qualifications
  • Bachelor's Degree in Nursing
  • 5 years of experience in an acute care hospital setting
  • Current RN license in New Jersey upon hire
  • Knowledge of care and utilization management
  • Strong interpersonal skills to manage case loads and communicate with multidisciplinary teams
