Job Summary
Collaborating across interdisciplinary teams, the full-time New Jersey Licensed Care Progression Manager will oversee patient care progression, ensuring timely discharge planning and appropriate post-acute services, while working in a hybrid environment with remote days on Tuesdays and Thursdays.
Key responsibilities
- Oversee patient populations to ensure timely progression of care and safe discharge planning using evidence-based criteria
- Apply clinical criteria to monitor medical necessity and reduce avoidable days in collaboration with healthcare teams
- Monitor length of stay, readmissions, and throughput metrics, contributing to initiatives that improve efficiency and patient outcomes
Required qualifications
- Bachelor's Degree in Nursing
- 5 years of experience in inpatient acute care hospital settings
- Active RN license in the State of New Jersey upon hire
- Strong knowledge of care and utilization management
- Ability to work independently and set priorities effectively
