Job Summary

A company is looking for a Clinical Pharmacist to manage pharmacotherapy outcomes for patients.

Key Responsibilities

• Act as a point of contact for patients, families, and healthcare providers regarding medication-related issues
• Ensure proper documentation and compliance in medication distribution and fulfillment
• Oversee Pharmacy Technicians and ensure adherence to medication management and regulatory guidelines

Required Qualifications

• BS in Pharmacy; PharmD preferred
• Active pharmacy license in the state of NJ
• 2+ years of experience in pharmacy distribution and chronic care management
• Preferred: Board Certified Geriatric Pharmacy Specialty Certification (BCGP)
• Must be registered or eligible for registration with the State Board of Pharmacy
