Job Summary

To support a growing virtual care team, the part-time Contractor Clinical Pharmacist will deliver pharmacist-led clinical services remotely, focusing on medication management for patients with chronic conditions and collaborating with interdisciplinary teams to enhance health outcomes.

Key responsibilities

• Conduct comprehensive medication reviews and reconcile medication-related issues
• Provide telephonic and virtual consultations to support patients with chronic conditions
• Collaborate with healthcare providers to develop individualized medication management recommendations

Required qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited institution
• Active New Jersey Pharmacist License in good standing
• Minimum of two years of experience in ambulatory care or related settings
• Strong computer proficiency and ability to navigate multiple clinical systems
• Ability to educate patients in Spanish language
