New Jersey Licensed Nurse Practitioner

Job Summary

To support a growing practice, the part-time New Jersey Licensed Nurse Practitioner will conduct comprehensive health assessments for enrolled Medicare Advantage members remotely, utilizing technology for patient-centered visits.

Key responsibilities

• Deliver preventive care and perform comprehensive reviews of members' medical histories and current health care needs
• Create personalized care plans in coordination with primary care physicians and actively close gaps in care
• Document progress notes and prepare member workups using Clover Assistant and EMR while coordinating with interdisciplinary care teams

Required qualifications

• Current unencumbered NJ Licensure as a Registered Nurse or valid New Jersey medical doctor license
• Certification as an Advanced Nurse Practitioner in specialty areas
• Active DEA and CDS registration
• Minimum of 1 year clinical work experience providing direct patient care
• Bachelor's degree (BSN) from an accredited school of nursing
